Here are some Important Steps For New Patients Prior To Your Appointment

1.)

Please have ready: Insurance card and personal form of ID such as social security number or drivers license with you. If you wear eyeglasses or contacts, Please bring your current or previous pair. Also, please bring your last eyeglasses or contact lens prescription. You can fill out our patient form electronically through our online scheduling system. If you decide to walk-in, please be ready to fill out our patient form when you arrive.

2.)

If you are going to be dilating, it is recommended that you bring someone else to drive you home. Dilating your eyes is an important part of a complete eye exam to check the health of the back of your eyes. Dilation is done by putting specialized drops in patient’s eye that widen your pupil for a short period of time. Due to these drops having side effects such as increased light sensitivity, blurred vision at near, and blurred vision at distance, it may be a good idea to bring someone with you to your appointment.

3.)

We also offer a unique exam option called “Optos” which will take a photo of the back of your eye without the side effects of a dilation. This is a good option for someone who does not wish to dilate or cannot dilate at the time of examination. Ask our staff for more information if you have any questions.
